• Perform comprehensive initial and ongoing patient assessments, developing personalized plans of care in collaboration with patients, families, and interdisciplinary teams.

• Provide skilled nursing services according to physician orders and monitor patient progress through timely clinical documentation.

• Supervise and support paraprofessional and professional care team members to ensure quality care delivery.

• Coordinate discharge planning and make appropriate referrals to other services as needed.

• Utilize clinical judgment and decision-making skills to manage an assigned caseload efficiently while maintaining high standards of care.

• Graduate of an accredited nursing program with a current Registered Nurse (RN) license in the practicing state(s).

• Two (2) years of current nursing experience preferred; one (1) year of management experience is a plus.

• CPR certification for adults, children, and infants; valid driver’s license and auto insurance if driving is required.

• Strong communication, organizational, and critical thinking skills; ability to prioritize multiple tasks and adapt to patient needs.

• Preferred experience in community health, home care, and broad medical-surgical knowledge; basic computer literacy is required.
